Application

Requirements

Applications must be mailed or brought into the centre in person. Applications cannot be faxed or done through the internet. Incomplete applications will not be accepted.

Each application must include:

2 passport-sized pictures, both signed on the back (see photo guidelines below)
A photocopy of your passport showing name, number, picture and expiry date
Payment for test fee
Addresses of institutions where you would like additional test results mailed (if applicable)
Postage charges (if applicable)

Photo Guidelines

You must submit photographs that are:

Identical
No more than 6 months old
Colour
Clear and in sharp focus
A minimum of 35mm wide by 45mm high
Professional on high quality paper

Your pose should be:

A close up of the head including: the top of the head and the top of the shoulders
Straight on to the camera
Both ears showing or the edge of the face visible
With a neutral expression (mouth closed, no smiling)
Without anything covering the face
Without the hair covering the eyes
Without glasses
(Head coverings for religious reasons are permitted)

The lighting and focus should include:

No shadows
Good colour balance showing the skins tones naturally
A plain background (white, beige or blue)
No objects or patterns in the background
No 'red-eye'

NB: Please do not use photo booths. London Drugs on Granville and Dunsmuir offers professional photos for approximately $14. We cannot reuse your photos from a previous test.
